What we provide

Support built for life in the community.

Legacy Point provides three complementary behavioral health services designed to strengthen daily functioning, emotional wellness, recovery, social connection, and continued independence. Services are individualized according to assessed needs, treatment goals, medical necessity, eligibility, and authorization requirements.

CPST

Community Psychiatric
Support and Treatment

CPST helps individuals strengthen coping, communication, organization, problem-solving, symptom management, and practical skills needed to function more successfully in everyday life.

Services are individualized and connected to documented treatment goals, functional needs, clinical recommendations, and appropriate oversight.

PSR

Psychosocial
Rehabilitation

PSR supports the development, restoration, and continued practice of skills that help older adults participate more fully in home, social, and community life.

Areas of support may include routines, self-advocacy, social connection, independent living, community participation, and confidence using learned skills.

PSS

Peer Support
Services

Peer Support connects individuals with someone who understands recovery through lived experience and can offer encouragement, perspective, practical assistance, and hope.

Peer support is grounded in mutuality, dignity, respect, self-determination, recovery, and the belief that no person should navigate behavioral health challenges alone.
Care that honors

the life already lived.

Older adults deserve support that respects their history, values, relationships, strengths, and personal goals. Our approach promotes well-being, reduces isolation, supports recovery, and helps each person continue building a meaningful path forward.

Dignity and Respect

Every person is treated with compassion, listened to, and valued for who they are.

Connection

We foster meaningful relationships and reduce isolation through consistent community engagement.

Recovery and Growth

Support strengthens confidence, resilience, independence, and the ability to live well each day.

Stability and Safety

We promote safe, supportive plans that encourage peace of mind and lasting stability.

Community First

Rooted in Wichita, we partner with families, providers, and community resources to support meaningful outcomes.

Founder story

A life of service.

Korleone Young is a Wichita native whose journey has taken him from local basketball courts to the professional stage and back home with a renewed purpose. His experiences as a professional athlete, mentor, and community advocate have shaped a deep understanding of resilience, discipline, human connection, and showing up for others.

Legacy Point Community Care represents the continuation of that vision: creating opportunities for older adults to heal, maintain meaningful relationships, strengthen daily-living skills, and remain connected to the communities they helped build.

Korleone’s leadership philosophy is grounded in the belief that healing is real, connection creates trust, lived experience can inform solutions, and service can become a pathway to hope.
